4 iv. Affidavit on bidder letter head as under I/We undertake and confirm that eligible similar works(s) has/have not been got executed through another contractor on back-to-back basis. Further that, if such a violation comes to the notice of Department, then I/we shall be debarred for bidding in CPWD in future forever. Also, if such a violation comes to the notice of Department before date of start of work, the Engineer-in-Charge shall be free to forfeit the entire amount of EMD or PG as per NIT Mandatory
5 v. GST registration Certificate as applicable (Uttar Pradesh state), if already obtained by the bidder. Or If the bidder has not obtained GST registration as applicable, then he shall scan and upload following undertaking along with other bid documents as per NIT Mandatory
6 vi. Similar work experience certificate as per criteria set in this NIT along with list of work as per proforma Annexure II given in the NIT. Similar work means Soil Investigation/Geo-Technical Investigation of soil. in Central Govt./Central autonomous Body/Central Public Undertaking/State Govt. Departments Mandatory
7 vii. Performance report of similar works as per Annexure III Mandatory
8 viii. Certificate for ERP Training, if already obtained by the bidder. Or If the bidder has not obtained ERP Training Certificate, then he shall scanned upload following undertaking along with the bid documents. If work is awarded to me, I/we shall obtain and submit an ERP Training Certificate, in case the agreement is executed through the ERP system of CPWD as per NIT Mandatory
